All these three phrases means younger sisters, but there are some slight difference. Both "妹妹" and "小妹妹" can be used in a flirting way, but the latter is offensive. “妹妹”just literally means "younger sister", while“小妹妹” means "youngest sister" who is under 10 and has a long age gap with the talker, and is sometimes used to refer to a woman's genital. “小妹”is usually used to call a woman who works in the service industry, especially a waitress.

小妹可能是家中最小的妹妹，在中国南方有些地区“小妹”这个称呼也用于年轻女性，不过不要随便用，因为有点调戏的意味。

@todayissunny I wouldn’t recommend it. It’s not a good idea to call a waitress 小妹. That would sound too personal, like you’re deliberately trying to make it seem like you and she are closer than you two really are.
Usually, Chinese people just call a waitress 服务员. This is a gender-neutral word, so you’re welcome to use it for a waiter.

@todayissunny 美女 can also sound flirty to women. Maybe some women have no problem being called that, but there are definitely some women who do mind.
小妹 in some situations is okay. For example, if I heard 小妹 said in costume dramas, I would think it was normal. Or you call your youngest sister 小妹, it’s also fine.
Anyway, 服务员 is the best way to call a waiter and a waitress, because it’s their job. You can never go wrong with it.
